Scanned High School Diploma
Scanned College Diploma (if applying for NY Board of Nursing)
Scanned Passport Copy
Soft copy of 2x2 picture or passport-sized picture in white background
Scanned Transcript of Records and Related Learning Experience
Scanned License
Birth Certificate (if applying for Northern Mariana Islands Board of Nursing) (original document)
Form 137 (if applying for Northern Mariana Islands Board of Nursing) (original document)
